What Are Hemorrhoids?
Piles or Hemorrhoids are basically varicose veins that swell up to form a mass of tissue around the anus. They cause a lot of discomfort and pain when not treated early enough. Some people develop them, others don't. It appears that some are just more susceptible than others.
The main culprit remains unidentified until today. Pregnancy can result in hormones that encourage the growth of hemorrhoids. So can advancing age which can weaken the connecting tissues between the anal linings and muscle underneath. But from clinical observation, stressing the anal linings can trigger the inner tissue to swell and protrude out of the anus. A stressed or constipated bowel movement encourages this.
Types
While hemorrhoids start out internally in the inner anal cushions, they tend to protrude and begin to show outside the immediate anal area. Hemorrhoids are generally graded according to severity. First degree piles are often temporary and can disappear right away.
Second degree piles have larger tissue mass around the anal area but often return back inside the rectal passage to become unobtrusive. It's the third and fourth degrees with tissue already protruding permanently from the anal passage that result in varying levels of pain, bleeding and overall discomfort and requiring some home medication regimen or surgery at its advance stage.

At Home
a) Prevent constipation. Frequent passing of hard feces stresses the anal muscles and cause hemorrhoids or worsen one if you already have it. Maintain a fiber-rich diet and drink plenty of liquids to soften the feces and make passing easy. Dietary supplements rich in fiber such as plantain and Psyllium seed husks or supplements with mild laxatives like lactulose can soften feces so the large intestines can pass them easily.
b) Topical application of astringents like witch hazel and soothing creams with corticosteroids such as Proctosedyl and Anugesic-HC, can relieve and shrink piles.
Non-Surgical
A couple of out-patient non-surgical treatment addresses the more advanced stages of hemorrhoids if home medication can't work. One clinical procedure called Banding inserts an elastic circular band above the hemorrhoid tissue to cut off its blood flow. Another called Sclerotherapy injects a solution into the hemorrhoid tissue itself. Both procedures will cause the hemorrhoids to shrivel and die and eventually fall off form the anus.
Surgery
This is the last resort when everything above fails.
a) Haemorrhoidectomy calls for the surgical removal of hemorrhoids and needs general anesthesia.
b) Stapled haemorrhoidopexy is a newer alternative surgical method that uses a special circular stapler that removes tissues above the hemorrhoid tissue cutting off this blood supply and effectively causes it to shrivel up and fall off.
c) Haemorrhoidal Artery Ligation Operation (HALO) calls for tying down the small arteries supplying blood to the piles and thus cause it to shrink.
